অনেক সময় চেম্বারে “ছোট মেয়ে বাবু” নিয়ে মায়েরা আসেন কিছু Gynaecology সমস্যা নিয়ে, তারা এই বিষয়গুলো নিয়ে খুব চিন্তিত থাকেন।
এসে বলেন
“ম্যাডাম আমার বাচ্চাটার কি হবে মেয়ে বলে কথা, আমি আগে খেয়াল করি নি /তেমন গুরুত্ব দেই নি /এখন দেখার সাথে সাথে নিয়ে আসলাম”।
তেমনি কিছু সমস্যা হল…….
1. Bleeding per vaginal after fews days of delivery
2. Enlarged breast (Unilateral /Bilateral)
3. Neonatal Leucorrhoea
4. Labial fusion/adhesion
আজ একটা বিষয় নিয়ে বিস্তারিত আলোচনা করলাম….
Labial Adhesion in Children
⚫Definition :
It is a condition where the two flaps of skin on either side of the opening to the vagina are joined together.
⚫Synonym :
- Labial fusion
- Labial Agglutination
⚫Causes :
এটা মনে করা হয় যে –
▪ Estrogen এর কম মাত্রা
▪ কড়া সাবান, ভেজা ডায়াপার, infection বা straddle injury এর মতো Trauma থেকে Valvur irrigation হতে পারে যার কারণে পরবর্তীতে Labia একত্রে লেগে থাকতে পারে।
⚫Time/Age:
Labial adhesion জন্মের পরে ৬ থেকে ৮ সপ্তাহ হিসাবে বিকাশ করতে পারে।
এছাড়া ৩ মাস থেকে ৬ বছর বয়সের মধ্যে বিকাশ করতে পারে। এটা Puberty অবধি স্থায়ী হতে পারে।
⚫How to Diagnose :
◾Look at the vulvar area and observe that the vaginal opening is either partially or fully covered by the labia with a white line indicating the adhesion.
◾When baby in sitting on a riding toy or having trouble passing urine, you may have discovered that her labia appears to be stuck together.
⚫Symptoms :
▪ Some have no symptoms
▪ Some have pain in the genital area
▪ Difficulty urinating
▪ Frequent UTI (Urinary tract infection)
⚫Treatment :
A) Most effective treatment is “Estrogen containing cream” applied twice daily for a period of time or until the adhesion is removed.
B) Surgery to open the adhesion is rarely done if the child was unable to pass urine.
রোগীর মা কে অবশ্যই এই Estrogen cream কিভাবে ব্যবহার করতে হবে এই নিয়ম আমাদের বুঝিয়ে দিতে হবে। শুধু Prescription এ লিখে দিলে হবে না, কারণ ফার্মেসীর দোকানদার কখনও এটা বুঝিয়ে বলতে পারবে না।
⚫How estrogen cream is applied to the labia?
1. Before applying cream give the baby a bath to remove any secretion and soften the labia.
2. Gently dry her vulva with a Soft towel.
3. Place the baby in frog-leg position.
4. Stand next to your child, facing her feet.
5. when her vulva is completely dry, squeeze a “small (pea size) amount of estrogen cream on your finger, use the other hand to gently separate the outer Labia(lips). Look at the white line where the labia are stuck together.
6. While keeping the outer lips separated with one hand, gently apply the cream to the white line. Avoid touching the anal opening.
7. Once the adhesion starts to separate, avoid putting the estrogen cream inside the vaginal opening.
8. Half of all adhesion will separate in 2-3 weeks. Sometimes it need to be continued for 1-2months.
◻ Notes◻
◾How to know that cream is working?
➡ Over the small openings will appear in the white line as the adhesion gradually disappears.
◾Is Estrogen cream safe?
➡ Yes safe, only a very small amount of cream for a limited amount of time. When adhesion is separated, then stop applying it.
◾Any side effect of Estrogen cream?
➡Too much estrogen cream can cause “estrogen effect” such as darkening of vulvar skin, breast buds, rare bleeding. Symptoms resolve after treatment is discontinued.
◾If Adhesion is open what rx is needed?
➡ Once the labia are separated, vaseline is used as an emollient to keep labia open for a few months and prevent irritation of the vulva area.
