তিতলি ও তার দাদাজানের E.coli নিয়ে আলোচনা-পর্ব ১

“আজ রবিবার” নাটকের তিতলি আর কঙ্কার কথা নিশ্চয় সবার মনে আছে। আর তাদের দাদাজান, যিনি সারাক্ষণ সবার খুঁত ধরতেন, সারাক্ষণ বিভিন্ন উপদেশ দিতেন এবং সারাক্ষণ কাশি দিতেন। তো একদিন কঙ্কাকে উপদেশ দিতে গিয়ে বললেন ” বল তো, নিশিতে প্রদীপ ভাতি এই কথাটায় ‘ভাতি’ মানে কি?” উত্তরে কঙ্কা বলল ” জানি না দাদাজান, আমি যেমন অনেক কিছু জানি না, তুমিও তেমন অনেক কিছুই জানো না! আচ্ছা, বল দেখি পৃথিবীর ওজন কত?”

দাদাজান শুনে অবাক হলো, পৃথিবীর আবার ওজন হয় নাকি! তাই গিয়ে তিতলি কে জিজ্ঞেস করলেন “পৃথিবীর ওজন কত তুই জানিস?” তিতলি পড়া নিয়ে খুব ব্যস্ত ছিল তাই বিরক্তি নিয়ে উত্তরটি দিল, এবং বলল ” আমাকে বিরক্ত করো না দাদাজান, দেখছোই তো পড়ছি।”

দাদাজান : তা কি নিয়ে পড়ছিস এত গভীর মনোযোগ দিয়ে? বইটা দে, একটু প্রশ্ন ধরে দেখি তোকে, কেমন কি পড়লি!

তিতলি : আমি ‘Escherichia Coli (E. coli)’ নিয়ে পড়ছি দাদাজান।

দাদাজান : ও বাবা, তাই নাকি? তো E. coli এর কিছু important properties বল তো?

তিতলি : বলছি, কিন্তু বলার মধ্যে ভুল ধরবে না কিন্তু দাদাজান। Properties গুলো হলো :

🔸 Gram staining property : E. coli is straight gram negative rod.

🔸 Morphology :
▪Motile with peritrichous flagella.
▪Most strain are fimbriated/ piliated.
▪It has 3 antigens : O (cell wall antigen), H (flagellar antigen), K (capsular antigen)

🔸 Facultative anaerobe
🔸 E. coli ferments lactose
🔸 Catalase positive
🔸 Oxidase negative

🔸Because there are more than 150 O, 50 H & 90 K antigens, the various combinations result in more than 1000 antigenic types of E coli.

🔸Specific serotypes are associated with certain diseases, e.g. O55 & O111 cause outbreaks of neonatal diarrhoea.

দাদাজান : এভাবে একটানা না বলে একটু থেমে থেমে বলতে পারিস না? যাইহোক, Reservoir of E. coli সম্পর্কে কি পড়লি, বল এবার?

তিতলি : কয়েকটা জিনিস জেনেছি, তোমাকে এবার থেমে থেমেই শোনাচ্ছি দাদাজান। সেগুলো হলো :
▪The reservoir of E. coli include both humans & animals.

▪The source of the E. coli that causes urinary tract infections is the patient’s own colonic flora that colonizes the urogenital area.

▪The source of the E. coli that causes neonatal meningitis is the mother’s birth canal ; the infection is acquired during birth.

▪In contrast, the E. coli that causes traveler’s diarrhoea is acquired by ingestion of food or water contaminated with human feces.

▪The main reservoir of enterohemorrhagic E. coli O157 is cattle & the organism is acquired in undercooked meat.

দাদাজান : আচ্ছা, একটি জিনিস ভেবে দেখেছিস? এত ছোট একটি ব্যাকটেরিয়া, কত রোগ ছড়িয়ে বেড়াচ্ছে! তোর পরের প্রশ্নে আসি, এবার E. coli এর virulence factors গুলোর নাম বল তো।

তিতলি : শোন দাদাজান, এটার নির্দিষ্ট কিছু components রয়েছে যেগুলো রোগ সৃষ্টিতে অবদান রাখে, যেমন :

🔹 Exotoxins ( enterotoxins) : 3 exotoxins –
▪Heat labile toxin (LT)
▪Heat stable toxin (ST)
▪Verotoxin

🔹 Pili : They help the bacteria to adhere to the site of infection.
🔹 Capsule : It interferes with phagocytosis.
🔹 Endotoxin
🔹 Verotoxin (Shiga like toxin, SLT)

দাদাজান : এই যে উত্তর দিলি, শুরুতেই একটি ভুল করেছিস, জানিস? ‘শোন দাদাজান’ আবার কি কথা? ‘জ্বী দাদাজান বলছি’ এমন বলতে পারিস না?
যাইহোক এবার বল, What are the diseases caused by E. coli?

তিতলি : জ্বী দাদাজান, বলছি। এবার ঠিক আছে?

দাদাজান : হুম, ঠিক আছে। এবার বল।

তিতলি : E. coli যেসকল রোগ সৃষ্টির জন্য দায়ী, সেগুলো হল :

🔺 Urinary tract infections (UTI) :
▪Cystitis
▪Pyelonephritis

🔺 Systemic Infections :
▪Wound infection
▪ Pneumonia, septicemia, abscess
▪ Neonatal meningitis
▪ Disseminated intravascular coagulation
▪ Biliary tract infection

🔺 Intestinal tract infections :
▪Diarrhoea
▪Dysentery

দাদাজান : খুব খারাপ হয়নি তোর পড়া তবে আমি একটু তোর বড় চাচার ঘর থেকে ঘুরে আসি, জেনে আসি পৃথিবীর ওজন মাপার জন্য এত বড় দাঁড়িপাল্লা আসলো কোত্থেকে!

তিতলি মনে মনে হেসে বলল, “আচ্ছা যাও দাদাজান, আমি বাকি পড়াগুলো সেরে ফেলি। ”
