N.meningitidis মূলত একটি Gram negative oval diplococci।
এটা দুই ধরনের disease এর জন্য দায়ী।
- Meningococcal meningitis
- Meningococcemia
Pathogenesis :
Meningococci প্রথমে nasopharynx এ colonize করে।

এবং upper respiratory tract এর একটি transient flora তে পরিণত হয়।
⬇
তারপর nasopharynx থেকে ছড়িয়ে পড়ে bloodstream এ।
⬇
Bloodstream থেকে ঢুকে পড়ে meninges এ।
⬇
Desseminated হয় পুরো শরীরে (meningococcemia)।
এই meningococcemia থেকেই develop করতে পারে Waterhouse Friderichsen Syndrome / hemorrhagic adrenalitis।
Definition:
This is an overwhelming systemic infection caused by N. meningitidis.
It is most commonly seen in persons suffering from deficiency of C5-C9 components of the complement.
Pathogenesis:
✔ Release of inflammatory cytokine (due to Lipopolysaccharide)
✔ Endothelial activation and injury :
N.meningitidis এর lipopolysaccharide endotoxin vascular damage করে। ফলশ্রুতিতে vascular leakage এবং tissue edema। এছাড়া প্রচুর পরিমাণে nitric oxide release হয়। Vascular smooth muscle relax হয়ে systemic hypotension develop করে।
✔ Induction of procoagulant state:
Vascular leakage এবং tissue edema এর জন্য small blood vessel গুলায় stasis হয়। Activated coagulation factor গুলা washout হতে পারে না। Thrombin activation হয়ে DIC develop করে।
✔ Generalized organ dysfunction :
পাশাপাশি বিভিন্ন organ এ blood flow কমে যায় মানে hypoperfusion। প্রথমদিকে body compensatory mechanism এ glucocorticoid এর release বাড়িয়ে দেয়। পরবর্তীতে DIC থেকে adrenal gland এ bleeding হয়ে adrenal insufficiency হয়। Adrenal gland এর synthetic capacity কমে যায় ultimately frank adrenal necrosis develop করে
Symptoms গুলা কি কি হতে পারে??
- High fever
- Features of shock
- Widespread purpura
- Hypotension
Laboratory Investigation :
• Complete blood count :
√ Thrombocytopenia : extreme
√ Leucopenia : poor prognostic sign (may be seen)
• Alternation in PT and aPTT
• ACTH stimulation test
• Blood culture
Treatment :
এটি একটি medical emergency। Appropriate antibiotic, vasopressor use এবং shock এর management করতে হবে।
