Let’s Learn about Primary Sjogren Syndrome & Secondary Sjogren Syndrome

Sjogren Syndrome Disease টা Female দের বেশি হয় আর Peak age onset হচ্ছে: 40 to 50 years।

এই Sjogren Syndrome এ আসলে কি হয়?

Lymphocytic infiltration of Salivary and Lacrimal Gland leading to glandular fibrosis and ultimately exocrine failure.

এই Disease টা অন্য কোন Disease process এর সাথে underlying থাকতে পারে। তখন একে আমরা বলি Secondary Sjogren Syndrome।

যেমনঃ
— Systemic Lupus Erythematosus

— Systemic Sclerosis/ Scleroderma

— Primary Billiary Cirrhosis (PBC)

— Myaesthenia Gravis

— Chronic Active Hepatitis
ইত্যাদি।

Presentation/ Clinical feature:

একটা Patient মূলত Present করবে —

1. Xeropthalmia (Dry eyes)
2. Blepharitis
3. Conjunctivitis
4. Filamentary keratitis

Mouth Involvement:

1. Dry mouth (Xerostomia)
2. Dental caries/ Dental infection

আর Skin Involvement এর জন্য হবে:

1. Xerosis (Dryness of skin)
2. Rash/ Irritation

এছাড়াও Fatigue, Non Erosive Arthralgia, Generalised Osteoarthritis ইত্যাদি থাকে।

আর less common কিছু clinical feature থাকে। যেমনঃ

1. Low Grade Fever
2. Cytopenia (anemia, leucopenia, thrombocytopenia)
3. Lymphadenopathy
4. Lymphoma
5. Interstitial Lung Disease
6. Renal Tubular Acidosis
   ইত্যাদি ।

Diagnosis:

এই Disease Diagnosis এর জন্য কতকগুলো test করা হয়ে থাকে যেমনঃ

1. Schirmer Tear Test: এখানে absorbent paper strip lower eyelid এ দেওয়া হয় যেটা Tear flow measure করতে পারে৷ In case of normal people more than 5mm wetting after 5 minute কিন্তু Sjogren Syndrome এ less than 5mm হয়ে থাকে।
2. Rose Bengal Score Test
3. If diagnosis is unclear then we can do Salivary Gland Biopsy: Infiltration of lymphocyte পাওয়া যাবে।
4. Autoantibody Screening
5. Rheumatoid Factor (RF) Test
6. Antinuclear Antibody (ANA) Test
7. Anti Ro/ Anti La Test

আর সাথে কিছু Routine investigation যেমন: Complete Blood Count (CBC), Erythrocyte Sedimentation Rate (ESR), Serum Electrolyte ইত্যাদি করা হয়।

Management/ Treatment:

এই Disease এর Treatment হচ্ছে Symptomatic।

★ Eye Symptom এর জন্য Lacrimal substitute, Contact lens ইত্যাদি।

★ যদি Oral Candidiasis হয় তাহলে তার Treatment হবেঃ
Artificial saliva spray, Saliva stimulating tablet, Chewing gum ইত্যাদি।

★ Early Disease Process এ glandular function improve করার জন্য Pilocarpine আবার যদি arthralgia হয় তাহলে Hydroxychloroquine দিতে হবে।

★ আবার যদি Renal involvement, lung involvement হয় তাহলে immunosuppressant দেওয়া হয় সাধারণত।

★ একটা Patient তার Generalised Lymphadenopathy + Salivary Gland Enlargement আছে তাহলে তার biopsy অবশ্যই করতে হবে malignancy exclude করার জন্য।
