Let’s Know about Graves Disease

ছবির মানুষটি একজন Graves disease এর রোগী।
এই পোস্টে Graves disease নিয়ে vital points গুলো আলোচনা করব।

Which type of disease?

It is an autoimmune disease usually due to TRAb (TSH receptor antibody).

এই antibody ২ রকম-
→stimulating
or,
→blocking type

Symptoms:
Graves disease এর রোগী
Thyrotoxicosis/ Hypothyroidism/ sometimes Euthyroid condition, যেকোনো presentation নিয়ে আসতে পারে।

If Trab is stimulating type
→presents as thyrotoxicosis.

If Trab is blocking type
→presentation is like Hypothyroidism.

তবে Maximum রোগী Thyrotoxicosis এর features নিয়ে আসে।

Special presentations of Graves disease:

→ Pretibial myxoedema

→Thyroid acropachy (rare)
(clubbing+ soft tissue swelling+
periosteal new bone formation)

→Graves opthalmopathy
(1. Opthalmoplegia,

2. Diplopia,

3. Lid lag,

4. Lid retraction,

5. Exposure keratopathy,

6. Corneal ulceration)

sclera বেশি exposed থাকে, তাই চোখে dryness, grittiness develop করে।

Investigations:

1. TSH, T3, T4
2. TRAb detection
   (major investigation for Graves)
3. Thyroid schintigraphy
4. Thyroid USG and if malignancy is suspected from USG, biopsy is taken.

Treatment:
যেহেতু Maximum রোগী Thyrotoxicosis নিয়ে আসে-

1. Antithyroid drugs
2. Subtotal thyroidectomy
3. Radio Iodine
