আজকে সকালে Burn unit এ একজন নতুন patient ভর্তি হয়েছেন। ডাঃ রাহনুমা আজ তার student দের কেইসটি দেখাবেন। ward এ গিয়েই রাহনুমা দেখলেন সব শিক্ষার্থীরা তার জন্য অপেক্ষা করছে। রাহনুমা patient এর দিকে তাকিয়ে তার শিক্ষার্থীদের উদ্দেশ্যে বললেনঃ
আজকে আমরা একটি বার্ন এর patient দেখবো। তার দেহের প্রায় ৭৫% পুড়ে গিয়েছে। একজন শিক্ষার্থী জিজ্ঞেস করলো বার্নটি কিভাবে হয়েছে ম্যাম?
ডাঃ রাহনুমাঃ গ্যাস সিলিন্ডার লিক করে রান্নাঘরে আগুন লেগে যায়।
আচ্ছা এখন তোমাদের মধ্যে থেকে কেউ একজন আমাকে বল Burn কাকে বলে?
আসিফ হাত তুলে বলতে থাকলোঃBurn is defined as dry heat injury caused by the application of flame and heated solid substance to the body.
ডাঃ রাহনুমাঃGood.এখন একজন বল Scald কি?
হাফসাঃ Scald is defined as moist heat injury produced by application of liquid at or near boiling point or in gaseous form such as steam to the body.
ডাঃ রাহনুমাঃ Good. Burn এবং scald এর মধ্যে কিছু difference রয়েছে। যেমনঃ
১.Burn সাধারণত যেই জায়গায়
heat apply করা হয়েছে তার উপরে হয় তবে Scald নিচে হয়।
২.Singeing of hair,sloughing,charning burn এ present থাকবে scald এ নয়।
৩.Burn এর skin scald এর তুলনায় thick থাকবে।
৪.Burn এ blister scald এর তুলনায় আগে তৈরি হবে।
৫.Burn এ contracture common scald এর তুলনায়
৬.Burn scald এর তুলনায় দেরিতে heal হয়।
৭.Burn এর fatality scald এর তুলনায় বেশি।
Burn আর scald এর difference টি written এর জন্য অনেক important.
এবার আসো Burn এর classification এ।
Wilson’s classification অনুযায়ী burn কে ৩ ভাগে ভাগ করা যায়।
1.Epidermal
2.Dermoepidermal
3.Deep burn
- Epidermal এ skin এ শুধু reddening and blistering দেখা যায়।এটি বেশ painful.
2.Dermoepidermal এ full thickness of skin এর destruction হয়। এটি আরও বেশি painful
এবং এখানে patient shock এ চলে যেতে পারে। - Deep burn এ tissue including subcutaneous tissue,fat,bones,muscles,nerve ending সব কিছু পুড়ে যায়। এটি painless হয়।
আমাদের এই patient এর দেহ প্রায় ৭৫% পুড়ে গিয়েছে। এখন আমরা কিভাবে হিসাব করতে পারি যে কতখানি পুড়ছে?

Head,neck & face এর জন্য 9%
Right handএর জন্য 9%
Left hand এর জন্য 9%
Front right side of trunk এর জন্য 9%
Front left side of trunk এর জন্য 9%
Back right side of trunk এর জন্য 9%
Back left side of trunk এর জন্য 9%
Right leg front এর জন্য 9%
Left leg front এর জন্য 9%
Right leg back এর জন্য 9%
Left leg back এর জন্য 9%
এবং
Genitalia এর জন্য 1% burn ধরে হিসাব করতে হবে।
যদি patient এর burn ৩০% এর কম হওয়ার পর ও patient মারা যায় তাহলে তার জন্য ডাক্তার responsible থাকবেন তবে burn ৩০% এর বেশি হলে তার জন্য ডাক্তার responsible থাকবেন না।
Burn এর medico-legal importance কেউ বলতে পারবে?
সাইদঃজ্বি ম্যাম।
1.Whether the burn is antemortem or postmortem.
2.Whether the burns are cause of death or accelerate the death.
3.The clothing as well as the body of the victim should be examined
4.Distinction between homicidal, suicidal or accidental burn.Accidental burn is most common.

1.Neurogenic shock
2.Hypovolemic shock
3.Toxemia
4.Sepsis
5.Hypokalemia
6.Acute renal failure
7.Gastrointestinal disturbance
8.Edema of the glottis
9.Fat embolism
10.Pulmonary embolism
Pugilistic Attitude কি কেউ বলতে পারবে?
তৌফিকঃজ্বি ম্যাম।Pugilistic attitude is a peculiar attitude produced in extensively burned body in which the posture of the body which has been exposed to great heat is often characterised by:
1.The legs are flexed at hips & knees.
2.The arms are flexed at elbows & wrists. Held out in front of the body.
3.All the fingers are hooked like claws
4.Head is slightly extended
5.Contraction of the paraspinal muscle often causes marked opisthotomas is an attitude commonly adopted by boxers.
ডাঃ রাহনুমাঃএর কারন কি?
তৌফিকঃThe stiffening is due to coagulation of protein of muscle.Dehydration causes contraction. The flexor muscle group being bulkier than the extensor contract more.
