Let’s know about Datura Poisoning

Lockdown এর এক অলস বিকেলে বারান্দায় বসে গল্প করছে জেবা আর জুই।
দুই বোনের মধ্যে জেবা বড়, মেডিকেলে পড়ে, MBBS 3rd year এ।
আর ছোট বোন জুই ক্লাস 9 এ পড়ে।

গল্প করতে করতে জেবা জুইকে তার পড়াশুনার ব্যাপারে জিজ্ঞেস করলো, “কেমন চলছে পড়াশুনা? আজকে কি পড়লো সারাদিন?”

জুই জানালো সে “হাজার বছর ধরে উপন্যাস” টা পড়েছে আজকে।
কথার প্রসঙ্গে জুই জেবাকে জিজ্ঞেস করলো- “আপু, হাজার বছর ধরে উপন্যাসে পড়লাম যে ছমিরন বিবি ধুতুরা ফুল খেয়ে মারা গিয়েছিলো, এইটা কি সম্ভব? তুমি কি জানো কিছু?”

জেবা – হ্যাঁ, সম্ভব তো।

জুই- কিভাবে সম্ভব? ফুল খেয়ে কেউ মারা যায় নাকি?

জেবা- তুই ছোট মানুষ, বললে কি বুঝবি এসব?

জুই- হ্যাঁ বুঝবো, বলো তুমি।

জেবা- তোকে নিয়ে কি যে করবো, এতো Curiosity তোর। আচ্ছা শুন তাহলে এইটা কিভাবে সম্ভব।

পুরো বাংলাদেশে 2 প্রকারের ধুতুরার প্রজাতি পাওয়া যায়।
(1) Datura fastuosa – গাঢ় বেগুনি রঙ্গের ফুল এবং এটি বিষাক্ত।
(2) Datura alba – সাদা রঙ্গের ফুল, এটি বিষাক্ত ফুল না।

জুই- আচ্ছা, তাহলে এই বেগুনি ফুলটা খেয়েই ছমিরন বিবি মারা গিয়েছিল?

জেবা- হ্যাঁ। আচ্ছা শুন.. (মনে মনে ভাবে, এই সু্যোগে আমার ধুতুরা Item টা Revise করা হয়ে যাবে 😎)

ধুতুরা হলো:

• Stupefying poison (এইটা খেলে মানুষ বোকামি শুরু করে)
• Deliriant poison (মানুষ আবোল-তাবোল প্রলাপ বলতে থাকে)
• Road poison

এখন এর Active principles গুলো শুন:

• Hyoscine (Scopolamine)
• Hyoscyamine
• Traces of atropine

Fatal dose:

• Powder seed- 0.6 to 1gm
• Crushed form- 100 to 125 seeds

Fatal period: 24 hours

জুই: এইগুলো কি বলো, কিছুই তো বুঝতে পারলাম না। আচ্ছা থাকুক এখন আর শুনবো না।

জেবা- আরে বোকা Sign symptoms টা তো শুনে যা।

জুই- Sign symptoms কি?

জেবা- Datura খেলে কি কি হতে পারে। যেমন:

• Dryness of the mouth & throat
• Difficulty in swallowing
• Dilatation of the pupil
• Diplopia
• Dry hot skin
• Drunkenness
• Delirium
• Drowsiness
• Dysuria
• Dysphagia
• Hallucination
• Rapid thready pulse about 120 to 140 per minute
• Ludicrous movement
• Morton sign
• Burning pain in the stomach
• Mental confusion
• Tachycardia
• Tachypnea
• Photophobia
• Death due to respiratory failure.

Circumstances of poisoning:
(1) Crushed powder used by criminals for stupifying a victim prior to robbery, rape or kidnapping (Road poison).
(2) Occasionally used for suicide.
(3) Homicide is rare.
(4) Used as abortifacient.
(5) Sometimes used for aphrodisiac action.

জুই: আচ্ছা আমি চলে যাই।

জেবা: এতো কিছু শুনলি, আর Treatment টা না শুনে চলে যাবি?
এইটা শুনে যা একটু..

Treatment:

• ABC management (Airway, Breathing and Circulatory management)
• Emetics can be used
• Washout the stomach repeatedly with a weak solution of tannic acid
• Give activated charcoal & a cathartic
• Wash out the lower bowel frequently
• Delirium can be controlled by bromides and short acting barbiturates
• Light diet
• Purgatives
• Catheterization in case of urinary retention
• Symptomatic treatment
• Specific Antidot:
  – Physostigmine 1 mg dose
  – Pilocarpine nitrate 5 mg dose

এখন শুন, এই ধুতুরা ফুল খেয়ে মারা যাওয়ার পর ছমিরন বিবির শরীরে কি কি change হয়েছিল-

Postmortem appearance of datura poisoning:

External:

• Signs of asphyxia

Internal:

• Fragment of datura seed may be present in the stomach. Mucosa is inflamed.
• Atropine can be found after long time.
• All internal organs are congested.
• Datura delays the putrefaction.

জুই: আর শুনতে চাই না আমি। আমাকে দয়া করে যেতে দাও।

জেবা: আর একটু শুন না। ধুতুরার বীজের মত আবার Capsicum এর বীজ আছে। দুইটাকে আলাদা করবি কিভাবে তুই?
জুই: কিভাবে করবো?

জেবা: এজন্যই তো শুনতে বলছি। তাহলে শুন এখন-

Datura seeds:

1. Size: Large and thick
2. Shape: Kidney shape
3. Colour: Yellowish brown
4. Smell: Odourless
5. Surface: Multiple small depression present.
6. Convex margins: Two ridges are present on convex margins
7. Taste: Bitter in taste.

Capsicum seed:

1. Size: Small and thin
2. Shape: Rounded shape
3. Colour: Pale yellow
4. Smell: Pungent, irritating smell.
5. Surface: Smooth surface
6. Convex margin: No doubt ridges on the margins.
7. Taste: Burning, irritating taste.

জুই: আমি আর শুনবো না বলেই দৌড়ে রুমে চলে গেলো।

জেবা এইবার মনে মনেই পড়তে থাকলো..

Morton sign গুলো কি যেন ছিল,
হ্যাঁ, মনে পড়েছে-

• Hot like ‘Hare’ : Rise temperature (even may rise as 107°F)
• Red like ‘Beet’ : Flushing of face followed by dilatation of cutaneous vessels.
• Blind like ‘Bat’ :
  – Conjunctiva suffused
  – Pupils dilated
  – Photophobia
  – Diplopia
  – Loss of accommodation of near vision
  – Temporary blindness
  – Drowsiness leading to coma
• Mad like ‘Hen’ :
  – May have low muttering delirium
  – Visual and auditory hallucination
  – Ludicrous movements
  – Drunken gait
• Dry like ‘Bone’ :
  – Mouth and throat will be dry
  – Intense thirst
  – Nausea
  – Vomiting
  – Dysphagia
  – Urinary retention

Ludicrous movement এর Another name হলো Pin rolling movement। In the phase of excitation of acute datura poisoning, ludicrous movement is observed.

It is characterized by-

• The patient tries to run away from bed.
• He picks up bed clothes.
• He tries to pull out the imaginary threads from the tips of a finger or threads of imaginary needles.
