স্যার : আজকে না তোমাদের Forearm এর Nerve supply টপিক টার উপর আইটেম নেওয়ার কথা।তোমরা সবাই পড়ে এসেছো?

স্যার : ঠিক আছে শুরু করি তাহলে।রোল ১ & ২ এসে পড়

স্যার : রোল ১ কে দিয়েই শুরু করি।বলত আমাদের Forearm কে কোন কোন Nerve Supply দেয়?
রোল-১:Median, & Ulnar nerve
স্যার: Forearm এ main nerve কোনটা?
রোল-১:Median nerve
স্যার : কেন?
রোল:-১ It supplies all the muscles of the front of the forearm except the medial half of the flexor digitorum longus and felxor carpi ulnaris
স্যার : তাহলে এই ২টা Muscle কে কোন Nerve supply দেয়?
রোল-১: ulnar nerve
স্যার : Median Nerve এর root value টা বল
রোল-১: Ventral rami of (C5- C8) segments of spinal cord
স্যার:আচ্ছা এবার রোল-২ Median Nerve এর Course টা বুঝিয়ে বলত?
রোল-২:
🔲At first,it enters the forearm via the cubital fossa.In the Forearm it travels
between flexor digitorrum profundus and flexor digitorum superficialis.
🔲Then it enters the palm deep to the flexor retinaculum

রোল-২: The branches of the median nerve is-
🔸Muscular Branches
🔸Cutaneous
branches
🔸Articular Branches
🔸Anterior Interosseous Branches
🔸Communicating Branches.
স্যার :Anterior Interosseous nerve কোন কোন Muscle কে supply দেয়?
রোল-২: It supplies the
▪️Flexor pollicis longus
▪️Lateral half of the flexor digitorum profundus.
It also supplies articular twigs to the inferior radio-ulnar, wrist and carpal joint
স্যার: খুব সুন্দর।এবার বলত Median Nerve এ কি কি injury হতে পারে?

◼️Injuries of the Median Nerve
★ Damaged at the Elbow:
✅Mechanism of injury: Supracondylar fracture of the humerus.
✅Motor functions: The flexors and pronators in the forearm are paralysed, with the exception of the flexor carpi ulnaris and medial half of flexor digitorum profundus. The forearm constantly supinated, and wrist flexion is weak (often accompanied by adduction, because of the pull of the flexor carpi ulnaris).
✅Flexion at the thumb is also prevented, as both the longus and brevis muscles are paralysed.
The lateral two lumbricals are affected, and the patient will not be able to flex at the MCP joints or extend at IP joints of the index and middle fingers.
Sensory functions: Lack of sensation over the areas that the median nerve innervates.
✅Characteristic signs: The thenar eminence is wasted, due to atrophy of the thenar muscles. If patient tries to make a fist, only the little and ring fingers can flex completely. This results in a characteristic shape of the hand, known as hand of benediction.
★ Damaged at the Wrist-
🔸Mechanism of injury: Lacerations just proximal to the flexor retinaculum.
🔸Motor functions: Thenar muscles paralysed, as are the lateral two lumbricals. This affects opposition of the thumb and flexion of the index and middle fingers.
Sensory functions: Same as an injury at the elbow.
🔸Characteristic signs: The hand is held in the same position as damage at the elbow, but the forearm is unaffected (not supinated or adducted, wrist flexion likely unaffected, depending on the location of the lesion).
স্যার:আচ্ছা, Median Nerve এর ক্ষেত্রে কি কি tests করা হয় বলতে পারবে?
রোল-২: জী স্যার পারব,
Tests গুলা হচ্ছে –
⬛Carpal tunnel syndrome – It can be performed during physical examination
⬛Tinel’s sign – tapping the nerve in the carpal tunnel to elicit pain in median nerve distribution.
Phalen’s manoeuvre– holding the wrist in flexion for 60 seconds to elicit numbness/pain in median nerve distribution.
স্যার : রোল-১ আমাকে এটার treatment গুলা একটু বুঝিয়ে বলত?
রোল-১ : স্যার Treatment গুলা হচ্ছে-
⚫Treatment of the primary cause-
- Splint in wrist, especially at night
- Local steroid injection (intra carpal space)
- Diuretic may help
- In severe case: decompressiom of carpal tunnel may be required
