Amytriptyline এর খুটিনাটি

Amitriptyline:
One kind of Anti depressive drug.
এটা market-এ tryptin 10mg নামে পাওয়া যায়।
এটা mainly TCA (Tricyclic Anti- depressive) drug।

Site of secretion:

একটা Synapse এ mainly দুইটা knob থাকে-

1. Presynaptic knob
2. Post synaptic knob
   এটা এই knob গুলোতে Noradrenaline and Serotonin এর breakdown বাড়িয়ে দেয় তখন এই knob গুলো থেকে Enzymes secration rate কমে যাবে। ACH (Achetylinecholine secration increases) হবে।

Indication of this drug:

1. Major depressive disorder: This like a persistent felling of sadness. It’s also called “Anhedonia”. সেই সময় এই drug apply করা হয়।
2. Tension headache (tightness felling on the head)
3. Migrane prophylaxis
4. Nocturnal enuresis (Involuntary urination while sleeping)
5. Diabetic Neuropathic pain

Side effects:
যেহেতু এটা Serotonin আর Noradrenaline level বাড়িয়ে দিচ্ছে তাই ACH level কমে যাবে।
Now come to the point-
What are the main function of ACH?
It helps in secration of salivary gland and Lacrimal gland, increase motility.

তাহলে কয়েকটা findings পাবো:

1. Dry eye, dry mouth
2. Constipation
3. Bradycardia
   These all are present when the subject take over dose of this drug.

এখন Patient এর Over dose হওয়ার পর কি কি check করা যায়?
Investigations:

1. Is the pupil dilated or not?
2. Pulse: Regular?
3. Air hunger breathing
4. ECG: large QRS complex
5. ABG (Arterial blood gas analysis) if over dose present তাহলে Patient এর Metabolic Acidosis present থাকবে।

How it can be compensated:
শুরুতেই বলা হয়েছে Patient এর metabolic acidosis থাকবে। Antidote একটাই সোডিয়াম বাইকার্বোনেট (NaHCO3)। এটা Acidosis কে neutralized করবে।
