শীতের সকাল,মিষ্টি রোদে উঠানের এক কোণায় বই নিয়ে বসেছে দীপা,কিন্তু কিছুতেই পড়ায় মন দিতে পারছেনা,দৃষ্টি তার ঠাকুরমার দিকে,রান্নাঘরের দরজায় মোড়া পেতে বসে খুকখুক করে কাশি দিচ্ছে,কেমন শুকিয়ে গেছে মনোরমা!খাবারেও রুচি কম,সন্ধ্যা করে জ্বরের ভাব হচ্ছে ইদানীং,আর কাশিটাও দীর্ঘদিন থেকে দেখা যাচ্ছে।
এরই মাঝে খোকন এলো,”কিরে দীপা,মাছ ধরতে যাবিনা আজ?”
দীপা: ঠাকুরমাকে নিয়ে খুব চিন্তায় আছি,জানিস? তোকে তো কাল বললাম ই! আচ্ছা শোন, “তোর বাবা মানে ডাক্তারবাবু বাসায় আছে এখন?”
খোকন: তা আছে, যাবি নাকি সাথে? গেলে চল এক্ষুণি।
ওদের বাসায় এসে দীপা ডাক্তারবাবুকে সব খুলে বলল। ডাক্তারবাবু শুনে কিছুক্ষণ চিন্তা করে বললেন, “মনোরমার সম্ভবত Tuberculosis/যক্ষা হয়েছে”।
দীপা: সেটা কি ডাক্তারবাবু?
ডাক্তারবাবু: যক্ষা/ Tuberculosis একটি সংক্রামক রোগ। ‘Mycobacterium tuberculosis’ নামক একটি জীবাণু এই রোগের জন্য দায়ী। ‘যক্ষা’ শব্দটা বাংলা ‘রাজক্ষয়’ শব্দ থেকে এসেছে।
দীপা: একটু আগ্রহী হয়ে জিজ্ঞেস করলো, আচ্ছা এই জীবাণুর নিজস্বতা কিংবা বৈশিষ্ট্য কেমন ডাক্তারবাবু? ডাক্তারবাবু বললেন, “হুম, বলছি শোন।”

দীপা: ঠাকুরমার যে লক্ষণ গুলা বললাম,এছাড়া আর কোন লক্ষণ নাই ডাক্তারবাবু?
ডাক্তারবাবু :মোটামুটি সব লক্ষণগুলি যদি বলতে চাই,তাহলে দাঁড়ায়-
✅ Sign & symptoms of tuberculosis
⭕ Coughing that lasts three or more weeks.
⭕ Coughing up blood.
⭕ Chest pain, or pain with breathing or coughing.
⭕ Unintentional weight loss.
⭕ Fatigue.
⭕ Fever.
⭕ Night sweats.
⭕ Chills
⭕ Loss of appetite
দীপা: আচ্ছা,এই রোগটা শুধুই ফুসফুসে হয় নাকি শরীরের যেকোন জায়গাতেই হতে পারে?
ডাক্তারবাবু: নাহ রে দীপা, ফুসফুস ছাড়াও শরীরের অন্যান্য অঙ্গপ্রত্যঙ্গেও হয়।
একটু পার্থক্য আছে,বুঝিয়ে বলি তোকে..
🗿একটা হলো Pulmonary tuberculosis আরেকটা Extrapulmonary tuberculosis.
Pulmonary TB: It’s an infectious bacterial disease characterized by the growth of nodules (tubercles) in the tissues, especially the lungs.এইটায় ‘Vaccine Protection’দিবে।
Extrapulmonary tuberculosis: Tuberculosis (TB) within a location in the body other than the lungs. This occurs in 15–20% of active cases, causing other kinds of TB.
🏀এটার Common Sites গুলা হলো- pleura, CNS, lymphatic system, genitourinary system, bones & joints. এটায়’ Vaccine Protection’ নাই।
দীপা: আচ্ছা বুঝলাম! কিন্তু ঠাকুরমার শরীরে যে এই রোগটা দেখা দিল,এটা কিভাবে বাসা বাঁধল ডাক্তারবাবু?
ডাক্তারবাবু :হাহাহা…তাতো একদিনে বাঁধে নাইরে মা,দিনে দিনে কিছু ধাপ পার করসে,ধাপগুলা বলি-
Pathogenesis of Pulmonary TB:
🎤Primary infection➡ It’s transmitted by inhalation of respiratory droplet➡ reach alveoli of lung& enter macrophages➡ bacilli survive & multiply within the phagosome➡ spread to the regional lymph nodes➡ lymphangitis➡ enlargement of hilar lymph node➡ primary Ghon complex. এটা Healed up & calcified হয়ে Ghon body ও তৈরী করতে পারে আবার Spread হয়ে দেহের অন্যান্য স্থানেও যেতে পারে।
🎤Secondary infection➡ Ghon complex & exogenous➡ T cell secrets lymphokine that activates macrophage➡ secretion of cytokines➡ anoxia & necrosis➡ vigorous pus formation➡ macrophage transforms into epithelioid cell➡ aggregation of it that form giant cell & fibrosis.
দীপা: এই প্রাথমিক আক্রান্ত, পরবর্তীতে আবারও আক্রান্ত ব্যাপারগুলা আলাদা নয় ডাক্তারবাবু?
ডাক্তারবাবু: হুম,ধরতে পেরেছো কিছুটা…দুইটার নির্দিষ্ট কিছু পার্থক্য আছে-
🔴Infection of an individual, lacking previous contact এটা হলো primary আর Infection that arises in a previously sensitized individual হলো secondary.
🔴Source Of infection➡Primary তে exogenous, secondary তে exogenous/endogenous
🔴Affected site➡primary তে lower part of upper lobe/upper part of lower lobe,secondary তে apical/ posterior segment of upper lobe
🔴Characteristic leision➡primary তে ghon complex & ghon focus,secondary তে tubercle formation.
🔴Cavitation+infectivity➡primary তে নাই,secondary তে আছে।
দীপা: বুঝলাম,কিন্তু ঠাকুরমার এই অবস্থার ওষুধপত্র, চিকিৎসা এসব নিয়ে চিন্তা হচ্ছে খুব!
ডাক্তারবাবু: এত চিন্তা করিস না তো,সব ঠিক হয়ে যাবে। আর আমি যাবো বিকালেই তোদের বাসায়, ব্যবস্থা হয়ে যাবে একটা কিছু।
এদিকে খোকন বড়শি নিয়ে প্রস্তুত, “কিরে দীপা, এবারতো চল,কয়টা মাছ ধরে আনি…”
