ফরেন্সিক ভাইভায় আর্সেনিক পয়সনিং

ফরেন্সিকের ভাইভার দিন সবাই ১০টার আগে কলেজে এসেই বই খাতা নিয়ে নড়াচড়া করছে। কারণ ১০ টার দিকে ভাইভা শুরু হওয়ার কথা। অংশু ৯.৫৫ তে এসে তাড়াতাড়ি করে বই দেখতে লাগল কারণ Poison বোর্ড এ প্রথমেই সে ঢুকবে। সে খুব nervous । Blood pressure বেড়ে গেছে। Sympathetic stimulation বেড়ে গেছে। এর মধ্যেই ডাক পড়ল অংশু এর। অংশু বোতল থেকে পানি খেয়ে রুমে ঢুকল।

1) The Fulminant type:

🔷All capillaries are markedly dilated especially in splanchnic area.
🔷It causes marked fall in blood pressure.

2) The Gastro enteric type:

🔶On GIT: →difficult in swallowing,
→constriction in the throat,
→ burning and colicky pain in oesophagus,
→ intense thirst

🔶On liver: Fatty infiltration.

🔶 On kidney: → oliguria,
→ uremia,
urine contains albumin, RBC, casts.

🔶On CVS: →acute circulatory collapse,
→ ventricular tachycardia.
→ventricular fibrillation.
👴: Acute arsenic poisoning নাকি cholera কিভাবে differentiate করবা?
👦: Acute arsenic poisoning এ pain in throat থাকবে vomiting এর আগে এবংCholera তে vomiting এর পরে।

👉Stomach wash- repeatedly with stomach tube & large amount warm water.
👉Freshly precipitated, hydrated ferric oxide : it forms inert ferric arsenate in gut
👉Antidote for absorbed poison is IM B.A.L.
👉Penicillamine: May be used with BAL(British anti-lewisite) – 100 mg/kg daily in 4 divided doses for 5 days.
👉Demulcents- lesion irritation.
👉Castor oil or magnesium sulphate- to prevent intestinal absorption.
👉Hemodialysis or exchange transfusion incase of renal failure
