সামনের সপ্তাহে thorax card final exam। আজকে স্যার সবার pending item নিবেন আগেই বলে দিয়েছিলেন।
স্যার: 1st rib item যার যার pending ছিলো আসো।
Item টেবিলে স্যারের সামনে বসে অাছে সাদিয়া এবং রিমা। তাদের 1st rib এর pending item ছিলো।
স্যার: Item ভালো করে পড়ে এসেছো তো?
“yes, sir.”
স্যার: ঠিক আছে। শুরু করা যাক। সাদিয়া, rib টা ঠিক করে ধরো এবং anatomical points বলো?
সাদিয়া:
This is the right sided 1st rib.
Anatomical points:
- Anterior end or sternal end is larger, thicker and pitted. Which lies anteriorly and two or three cm below the posterior end.
- Posterior end is small and round lies posteriorly.
- The external border is convex, thick posteriorly and thin anteriorly, has no costal groove.
- Superior surface of the shaft is marked by two shallow oblique grooves.
- Internal border is concave and thin and it is marked by scalene tubercle near the mid point.
স্যার: এবার রিমা, 1st rib এর identification point বলো?
রিমা:
Identification point :
- The shaft is not twisted.
- Most acutely curved.
- Shortest and broadest of all ribs.
- No costal groove.
- Broad and flat from above downwards.
- A single facet on the head.
- Internal border is marked by scalene tubercle.
- The superior surface of the shaft is crossed obliquely by two shallow grooves, seperated by a slight ridge.
স্যার: সাদিয়া, neck এর relations বলো?
সাদিয়া:
Anterior relations of the neck:
(Medial to lateral)
- Sympathetic chain.
- Superior intercostal vein.
- Superior intercostal artery.
- Ventral ramus of T1 spinal nerve.
স্যার: ঠিক আছে। Inner border এর attachment বলো?
সাদিয়া: Suprapleural mambrane.
স্যার: Suprapleural mambrane এর another attachment কোথায় থাকে?
সাদিয়া: চুপ করে আছে।
স্যার: কী হলো?
সাদিয়া: মনে পড়ছে না স্যার।
স্যার: Pending item দিতে আসছো তারপরও এই অবস্থা! রিমা, তুমি বলো?
রিমা: Tip of the transverse process of the 7th cervical vertebrae.
স্যার: Superior surface of the shaft এর groove এর attachment এ কী থাকে? সাদিয়া বলো। আগের question তো পারনি, এটা না বলতে পারলে আবার pending দিবো কিন্তু।
সাদিয়া:
• Anterior groove: subclavian vein.
• Posterior groove: subclavian artery and lower trunk of brachial plexus.
স্যার: Superior surface এর muscle attachment বলো?
সাদিয়া:
The superior surface of the shaft gives attachment to:
- Scalenus anterior: is inserted into the scalene tubercle.
- Scalene medius: is inserted into a large rough area on the superior surface, behind the groove for subclavian artery.
- Subclavious: arises from the more anterior area of the superior surface in front of the groove for subclavian vein.
- Costoclavicular ligament: is attached to the rough area between groove for subclavian vein and the first costal cartilage.
স্যার: রিমা, 1st rib এর articulation বলো?
রিমা:
• Costocondral joint primary cartilaginous joint.
• Costovertebral joint plane variety of synovial joint.
• Costotransverse joint plane variety of synovial joint.
স্যার: Ossification কী হবে?
রিমা: Cartilaginous ossification.
স্যার: সাদিয়া, Flat bone এর ossification কী?
সাদিয়া: Membranous ossification.
স্যার: Ribs and sternum is flat bone. আমরা জানি flat bone এর membranous ossification কিন্তু sternum and ribs falt bone হলেও cartilaginous ossification।
তোমরা যাও।
“Next আসো। যাদের 2nd rib pending ছিলো।”
