🧛‍♂️ The Vampire disease 🧛‍♂️

স্কুলে থাকতে ‘Twilight Saga’/ ‘The Vampire Diaries’ দেখে রিয়েল লাইফে Vampire এর সাক্ষাৎ পেতে চায়নি এমন মানুষ খুঁজে পাওয়া ভার। আদতে এমন মানুষরূপী Vampire খুঁজে পাওয়া না গেলেও আজ থেকে প্রায় ১৫০ বছর আগে এমনই একটি রোগের সন্ধান পাওয়া যায় যার কিছু সিম্পটম হুবহু Vampire দের মত বৈকি!!

The Vampire disease/ Porphyria মূলত একটিঃ

‘Genetic Disorder’ যেখানে মানবদেহের বিভিন্ন অংশে Porphyrin নামক একটি রাসায়নিক উপাদান অস্বাভাবিক হারে জমতে শুরু করে।

💠 কেন হয় এমনটি? 

▶ আমাদের শরীরের RBC তৈরির একটি অত্যন্ত গুরুত্বপূর্ণ অংশ হচ্ছে হিম। হিম নিজে আবার তৈরি হয় Porphyrin থেকে। হিম সিন্থেসিস অর্থাৎ হিম তৈরির পাথওয়েতে আটটি এনজাইম ক্রিয়াশীল থাকে, যাদের মধ্যে চারটি থাকে মাইটোকন্ড্রিয়ায় আর অপর চারটি থাকে সাইটোসলে। এই এনজাইম পাথওয়ের কোথাও যদি জেনেটিক্যাল ডিফেক্ট থাকে তবে এই Porphyrin আর হিমে রূপান্তরিত হয় না, এর পরিণতিতে শরীরে Porphyrin জমতে শুরু করে। 

💠 এর সাথে Vampire এর কী সম্পর্ক? 

▶ Porphyria এর দুইটি সাবটাইপ-

১. Acute Porphyria (এখানে Porphyrin জমা হয় Nervous System এ)

২. Cutaneous Porphyria (এখানে Porphyrin জমা হয় Skin এর নিচে, বলে রাখা ভালো এটিই সবচেয়ে কমন)। 

🔵 নামকরণঃ

▶ Vampire রা যেমন দিনের আলোয় বের হতে পারে না, তেমনিভাবে ‘Cutaneous Porphyria’ এর Patient দের sunlight exposure এ skin blistering হয়। সেখান থেকেই এই নামের উদ্ভাবন।

🔴 স্থায়িত্বঃ

▶ পুনরাবৃত্তিক আক্রমণ ঘটে যা কয়েকদিন থেকে সপ্তাহ পর্যন্ত স্থায়ী হতে পারে।

🔴 কী কী Symptoms নিয়ে আসতে পারে?

▶ 1. Acute Porphyria:

🔷 Seizure,

🔷 Chest/ Abdominal pain,

🔷 Vomiting,

🔷 High blood pressure,

🔷 High heart rate.

▶2. Cutaneous Porphyria (most common):

🔷 Skin blistering in sunlight exposure,

🔷 Overgrowth of facial hair,

🔷 Urine discoloration,

🔷 Brownish teeth.

🔵 কী কী উপায়ে রোগটিকে diagnose করা যেতে পারে?

🔶 Blood test,

🔶 Urine test,

🔶 Stool test,

🔶 Genetic testing/ DNA testing (as it’s a rare genetical disease)

🔴 কী কী complications দেখা দিতে পারে?

🔷 Paralysis,

🔷 Low blood sodium levels,

🔷 Seizures. 

🔘 Treatment কী দিবো?

▶ It actually depends on type (Porphyria cutanea tarda is believed to be the most common type) & symptoms. Treatment of porphyria of the skin generally involves the avoidance of sunlight, while treatment for acute porphyria may involve giving intravenous heme or a glucose solution. Rarely, a liver transplant may be carried out.

💠 দুষ্প্রাপ্যতাঃ

▶ বর্তমান বিশ্বে এই রোগটি এতোটাই ‘Rare’ অর্থাৎ দুষ্প্রাপ্য যে reported patient এর সংখ্যা মাত্র ১৫০ জন।
