সাদিয়ার আজ ফরেন্সিক মেডিসিন এর টার্ম এর ভাইভা, তাই সে খুব টেনশনে আছে, একটু পরেই তার ডাক পড়বে। Weapon বোর্ড এর ভাইভা টা হবে পরে আগে তার Poison বোর্ড এর ভাইভা। হঠাৎ ওর ডাক পড়ল, তারপর সে Viva দিতে চলে গেল।
কি হলো এরপর আসুন জেনে নিই-
সাদিয়া: আসসালামুআলাইকুম Sir.
স্যার: বসো। কি নাম তোমার মা?
সাদিয়া ভয়ে ভয়ে উত্তর দিলো, “সাদিয়া, স্যার”।
স্যার: কি হলো এতো ভয় কিসের? ভয় পেলে এক্সাম ভালো হবে না কিন্তু। আর আমরা এই সব খুব অপছন্দ করি। ভয় পেয়ো না কেমন?
সাদিয়া : আচ্ছা sir.
স্যার: দেখি, নাও কোনটা নিবা।
তারপর সাদিয়া কেরোসিনের বোতল হাতে নিয়ে বলা শুরু করল , “Sir, This is a clear glass bottle with stopper containing light blue colored fluid with typical smell.It is kerosene Oil”
স্যার: Kerosene oil কি?
সাদিয়া: Kerosene oil is an organic petroleum derived fuel.
স্যার: Kerosene এর source কি?
সাদিয়া: Sir, Crude petroleum.
স্যার: Fatal dose বলো।
সাদিয়া: Sir,
• Adult: 30-100 ml
• Children: 20-30 ml
স্যার: Kerosine এর রঙ নীল কেন?
সাদিয়া: আসলে স্যার, Kerosene is colorless but to make it identifiable it is colored blue. This coloration also prevents adulteration.
স্যার: How kerosene oil is obtained?
সাদিয়া: By fractional distillation of crude oil or petroleum.
স্যার: Pure kerosene কোথায় use হয় জানো?
সাদিয়া: Airplane fuel হিসেবে।
স্যার: Petroleum derivatives কি কি আছে জানো?
সাদিয়া: Gasoline, naphthalene, paraffin etc.
স্যার: Kerosene দিয়ে mainly কি ধরনের poisoning হয়ে থাকে?
সাদিয়া: Accidental poisoning.
স্যার: Accidental kerosene poisoning কাদের ক্ষেত্রে common?
সাদিয়া: হামাগুড়ি দেয়া শিশু (within 1year)
স্যার: শিশুদের accidental poisoning কিভাবে হয়?
সাদিয়া:মেঝেতে পড়ে যাওয়া kerosene চুষে চুষে হতে পারে বা হারিকেনের সলতে চুষে চুষে ও হতে পারে sir.
স্যার:আচ্ছা, Kerosene আমাদের দেশে কি কাজে ব্যবহার হয়?
সাদিয়া: হারিকেন জ্বালাতে, রান্নার কাজে চুলা জ্বালাতে।
স্যার: এখন বলো, mode of action অনুযায়ী kerosene কি ধরনের poison?
সাদিয়া: Organic irritant poison
CNS depressant poison
স্যার: Kerosene এর medico-legal importance বলো।
সাদিয়া: Mainly accidental. Sometimes, used for self immolation( suicidal-rare), Dowry death (homicidal) যৌতুকের টাকা না পেয়ে অনেক সময় গায়ে আগুন লাগিয়ে দিয়ে হত্যা করে থাকে।
স্যার: Kerosene poisoning এর route কি কি?
সাদিয়া:
- Ingestion
- Inhalation of fume
- Aspiration to lung
স্যার: কোন organ বেশি ক্ষতিগ্রস্ত হয় বলতে পারবে?
সাদিয়া: Sir, lungs.
স্যার: Lungs এ কি কি complication হতে পারে?
সাদিয়া: Chemical pneumonitis
Pulmonary oedema
Bronchopneumonia
স্যার: Kerosene এর কি কি effect আছে?
সাদিয়া: Local effect আর remote effect.
স্যার: Example দাও.
সাদিয়া: Local হলো lungs এবং GIT তে irritation করে
আর remote হলো CNS depression করে।
স্যার: CNS depressant আর কি আছে?
সাদিয়া: স্যার barbiturate, morphine, alcohol
স্যার: What are the clinical features of kerosene poisoning?
সাদিয়া:
- Kerosene smell
- Burning sensation in throat
- Colicky Abdominal pain(due to distension of stomach)
- Nausea
- Vomiting
- Diarrhoea
- Cough
- Labored breathing
- Increased respiratory rate
- Respiratory distress
- CNS- drowsiness
- Convulsion
স্যার: Respiratory distress কেন হয়?
সাদিয়া: Sir because of bronchopneumonia and pulmonary edema.
স্যার: Treatment of kerosene Oil poisoning বলো।
সাদিয়া:
- Removal of cloths and thorough wash on area of contact
- Oxygen inhalation in case of respiratory distress
- IV fluid replacement in case of dehydration
- Oral administration of Liquid paraffin-which slows absorption
- Antibiotic( penicillin) in case of aspiration pneumonia
- Steroid for chemical pneumonitis
- Diuretics –>Frusemide for pulmonary oedema
স্যার: Gastric lavage দিবা না?
সাদিয়া: না sir.
স্যার :কেন?
সাদিয়া: There is a chance of aspiration pneumonia.
স্যার: Postmortem findings কি পাবা externally?
সাদিয়া: যেহেতু respiratory distress হয়, cyanosis and congestion of face আর এর সাথে Kerosene এর গন্ধ তো থাকবেই
And may be discharge of blood stained froth from mouth and nose
স্যার : হুম, ঠিকাছে যাও।
তারপর সাদিয়া খুশিমনে বের হয়ে চলে গেল।
এখন সে কিছুটা নির্ভয়ে বাকি Weapon বোর্ড এর ভাইবার জন্যে রিভিসন দিবে,কারন Poison বোর্ডটা ভালই হয়েছে তাই ভয় কেটে গিয়েছে তার।
