ইন্টার্নশিপ শুরু করার পর এই ব্যাপারটা নিয়ে ঝামেলায় পড়তে হয়নি এমন কাউকে দেখিনি। যে কাজটা কোনদিন করিনি, পড়িওনি কিভাবে করতে হয়, হুট করেই সেটা করতে নামায়ে দেওয়া হয়। সিনিয়রদেএ দেখে আমরা কবিরাজি পদ্ধতিতে ডেথ ডিক্লেউয়ার করা শিখে যাই। কিভাবে? সিনিয়ররা বলবেন- “তিনবার ফলো আপ দিবা ১০/৫ মিনিট পর পর। পালস, বিপি, পিউপিল দেখবা। তার ১৫ মিনিট পর একটা ইসিজি করবা।” বিশ্বাস করেন ভাই, পৃথিবীর কোন বইয়ে মরে গেছে কিনা বুঝার জন্যে প্রেশার দেখার কথা বলা নাই। চলেন সঠিক উপায়টা শিখে নেই।
আগেই বলে রাখি,
★Death দুইভাবে হতে পারে।
✓প্রথমত Death following irreversible cardio-respiratory arrest,
✓দ্বিতীয়ত Death following irreversible cessation of brainstem function ( in comatose patients).
এই পোস্টে আমরা আলোচনা করবো Irreversible cardio-respiratory failure এর জন্যে মৃত্যু হলে সেটা কিভাবে declare করতে হয়।আমাদের ওয়ার্ডে এটাই দরকার হয়।
★Death following irreversible cessation of cardiorespiratory function আমরা দুই ধাপে confirm করি:
– Confirmation of irreversible
cardiopulmonary arrest
– Confirmation of absence of brainstem activity.
★Irreversible Cardiorespiratory Arrest:
***Irreversible cardiorespiratory arrest কনফার্ম করার জন্যে কমপক্ষে ৫ মিনিট ধরে examine করতে হবে। নিচোর জিনিসগুলো দেখে আমরা আমরা confirm করে থাকি:
– দুইটা আলাদা জায়গায় Heart Sound শুনে দেখতে হবে। প্রতি জায়গায় ১ মিনিট করে মোট ২ মিনিট
– কোন একটা central artery তে ( Carotid or Femoral ) pulse দেখতে হবে ১ মিনিট ধরে
– দুইটা আলাদা জায়গায় breath sound শুনে দেখতে হবে। প্রতি জায়গায় ১ মিনিট করে মোট ২ মিনিট।
***NB : A code of practice for the diagnosis and confirmation of death এ বলা হয়েছে যে cardiorespiratory arrest confirm করার জন্যে জন্যে উপরের তিনটা জিনিস এর পরিবর্তে ECG, Intra-arterial pressure monitoring কিংবা Echocardiography এগুলোর যেকোন একটা বা একাধিক প্রমাণ হলেও চলবে। বইয়ে লেখা এভাবে :
……..their use can be supplemented in the hospital setting by one or more of the following:
– asystole on a continuous ECG display
– absence of pulsatile flow using direct intra-arterial pressure monitoring
– absence of contractile activity using echocardiography
যাইহোক, আগের কথায় ফেরত যাই এবার। এগুলো কোনটা দেখার সময় যদি কোনটা পাওয়া যায়, অর্থাৎ ধরেন আপনি breath sound শুনতে পেলেন অথবা pulse পেলেন, তাহলে ওখানেই থেমে যেতে হবে এবং ৫ মিনিট পর আবার এই ৩ টা জিনিস একই নিয়মে দেখতে হবে শুরু থেকে। যতক্ষণ পর্যন্ত ৩টা জিনিসই absent না হবে ততক্ষণ পর্যন্ত ৫ মিনিট পরপর দেখতে থাকবেন। যদি প্রথমবারেই দেখেন যে Heart sound, Pulse এবং Breath Sound নাই, তাহলে Congratulations ! You have confirmed irreversible cardiorespiratory arrest. এবার নিচের step এ যান।
★Brainstem Activity :
বাম হাতের thumb আর index finger দিয়ে দুই চোখের পাতা একসাথে উপরের দিকে তুলুন ( Jaundice যেভাবে দেখে সেভাবে )।
– Direct এবং Consensual light reflex দেখেন।
– Corneal Reflex দেখুন।
– Supra orbital notch এ firm pressure দিয়ে কোন motor response আছে কিনা দেখুন।
সবকিছু দেখা শেষ হলে ঘড়ি দেখবেন। এখন ঘড়িতে যে সময় দেখাচ্ছে সেটাই time of death. সবশেষে রোগীকে ঢেকে দিন । Cover the patient in a dignified manner. সবশেষে examine করে যা যা পেলেন, লিখে রাখেন। লিখে রাখাটা জরুরি।
★Documentation:
– Documentation করা খুব জরুরি। যা যা পেলেন সব লিখে রাখবেন।কিভাবে লিখবেন তার একটা নমুনা নিচে দেওয়া হলো :
– Heart Sound : Not audible
– Breath Sound : Not audible
– Pulse : Non-recordable
– Pupil : Fixed , dilated, unreactive to light
– Corneal Reflex : Absent
– Motor Response : Absent
[ Formal signature ]
The patient died on 12.4.2020 at 7:03 am.
★At a Glance:
– Auscultate for Heart Sound on 2 seperate places. 1 minute in each space, total 2 minutes.
– Palpate Carotid/Femoral Pulse for 1 minute.
– Auscultate for Breath sound on 2 seperate places. 1 minute in each space, total 2 minutes.
– Check the Direct and indirect light reflex
– Check the Corneal reflex
– Close the eyelids.
– Check motor response by applying firm pressure on Supraorbital notch.
***If any of the above finding is positive, wait for 5 minutes and check again. When all the findings are negative, go to the next step.
– Cover the patient in a dignified manner.
– Check the time. This is the time of death.
– Documentation of the findings.
– Do not forget to mention the date and time of death.
– Your full name, qualification, designation with formal signaturebelow the death note.
– Ask one of the patients attendant. ( male, preferably ) to follow you.
– Go to a safer place , where other healthcare workers are present. ie doctors room, infront of nurses table etc. Declare death to the patients attendant with full empathy.
***NB: Macleods Clinical Examination 14th edition এ heart sound, lung sound ১ মিনিট ধরে দেখতে বলার কথা হয়েছে। A code of practice for the diagnosis and confirmation of death এ আলাদা করে সময় ভাগ করা নেই। ওখানে বলা হয়েছে cardiorespiratory failure হয়েছে কিনা এটা দেখতে হবে ৫ মিনিট ধরে।
