Murphy’s sign: (American physician, John Benjamin Murphy সর্বপ্রথম এই sign describe করেন, তাই তার নাম অনুসারে এর নাম Murphy’s sign নামকরণ হয়)
What is Murphy’s sign?
- যখন right subcostal margin এর just নিচে হাত রেখে, patient কে deep inspiration নিতে বলা হয়, তখন sharp pain হয় (Sharp pain এর কারণে breathing pattern ঐ সময় আটকে যায়, এই breathing pattern আটকে যাওয়াকে Catch hold breathing বলে)
What are the causes?
- Acute Cholecystitis ( গ্রিক cholecyst মানে gallbladder, আর itis মানে inflammation। তাহলে বুঝা যাচ্ছে, gall bladder এর acute inflammation কেই acute cholecystitis বলে)
কিভাবে Acute cholecystits এ Murphy’s sign positive হয়?
- দেহের Right side এ liver থাকে, liver এর নিচেই থাকে gall bladder। আর liver এর উপরে থাকে right lung। যখন deep inspiration (জোরে শ্বাস) নেওয়া হয়, তখন lung ফুলে উঠে। এখানে deep inspiration খুবই important। কারণ, deep inspiration এর কারণেই lung বেশি ফুলে উঠে, যে কারণে lung তার নিচে থাকা liver কে চাপ দেয়, আর liver চাপ দেয় liver এর নিচে থাকা gall bladder কে। এই চাপের কারণে gall bladder নিচে নেমে যায়। নিচে নেমে যাওয়ার কারণে gallbladder right subcostal এর নিচে থাকা হাতের উপর এসে চাপ খায়, তখন inflammed gallbladder, এই চাপের কারণে sharp pain এর সৃষ্টি করে। একেই বলে Murphy’s sign!
