আজ আমরা জানবো Tetanus, neonate দের কেন হয় এবং এ থেকে প্রতিকার এর উপায় কি।
Tetanus neonatorum কেন হয়?
- Due to septic cutting of umbilical cord by unsterilized instruments.
Agent of tetanus?
- Clostridium tetani.
Tetanus neonatorum এর ক্লিনিক্যাল ফিচার গুলো কি কি?
- Rigidity of muscles with spasms
- Irritability (continuous crying)
- Grimacing of face & restlessness
- Poor feeding / suckling ability
Neonatal Tetanus নিয়ে শিশু জন্মানোর ০৮ দিন পরপরই symptoms গুলো প্রকাশ পেতে শুরু করে। তাই এর যে কোন একটি symptom লক্ষ্য করলে দ্রুত চিকিৎসকের পরামর্শ নেওয়া উচিৎ।
এখন আমরা Neonatal Tetanus এর Risk factors গুলো সম্পর্কে জানবো-
- Unvaccinated pregnant mother
- Unhygienic conditions during child birth
- Animals / livestock kept near the vicinity of home
- Use of traditional methods for umbilical cord management (rat faeces, ash, herbs)
- Unclean hands and unsterilized instruments
- Newborn exposure to raw soil / dust
- Lack of awareness of symptoms / access to primary health care
- Traditional practices like circumcision আমরা কিভাবে এটাকে prevent করতে পারি?
- মা কে immunized করার মাধ্যমে। যেমনঃ
আমাদের দেশে ১৫ বছর বয়স থেকে মেয়েদের TT vaccine দেওয়া হয়, সেইটার schedule হলোঃ
TT1 = At first exposure but not before 15 years ( gives no immunity)
TT2 = After 1 month ( gives immunity for 3 years)
TT3 = After 6 month ( gives immunity for 05 years)
TT4 = After 1 year ( gives immunity for 10 years)
TT5 = After 1 year ( gives life long immunity)
Pregnancy period এ কোনো মা যদি previously immunzied থাকে with 5 TT vaccine, তাহলে তাকে one booster dose of TT দিতে হবে।
এখন কোনো pregnant women যদি এসে বলে যে, সে কোনো ধরনের TT vaccine নেয় নি, সেক্ষেত্র তাকে 2 dose of TT vaccine দিতে হবে।
TT1 = at 16-20 weeks
TT2 = at 20-24 weeks of pregnancy
TT vaccine should not be administrated in 1st trimester of pregnancy because rarely there is possibility of abortion. So it is given after 3 month.
আবার কোনো pregnant women যদি delivery moment এ এসে বলে যে, সে immunzied না, সেক্ষেত্রেও তাকে TT & TIG দিয়ে delivery continue করতে হবে।
But after delivery কেউ যদি বলে সে immunized ছিলো না, তখন baby কে within 6 hours এর মধ্যে anti-toxin দিতে হবে।
এখন আসি immunized mother with full doses of TT vaccine মা, আবার কখন TT vaccine নিবেন or নিবেন না during her pregnancy period:
If pregnancy occurs within 3yrs after given TT vaccine, then no needs.
After 3 years = 1 dose of TT vaccine
After 5 years = 2doses of TT vaccine
এছাড়া during delivery period এ 3C অবশ্যই maintain করতে হবেঃ
C = Clean hands of the birth attendants
C = Clean delivery surface
C = Clean cord care
That’s it.
