Zollinger-Ellison Syndrome (American surgeons Zollinger and Ellison প্রথম এই disease টি কে describe করেন তাই নাম Zollinger-Ellison Syndrome)।
এই Disease টি কেন হয়?
যদি কারো Gastrinoma (gastrinoma মানে যে cell থেকে gastrin hormone উংপন্ন হয় সেই cell-এর Tumor) থাকে তাহলে তাদের এই Disease টি হয়।
কেন হয়?
কারণ, আমরা জানি Gastrin হরমোন এর কাজ হলো, এটি parietal cell এর উপর কাজ করে HCL উৎপন্ন করা, এখানে gastrinoma এর কারণে excess Gastrin hormone উৎপন্ন হবে।

এই Excess gastrin hormone বেশি বেশি parietal cell কে stimulate করবে এবং বেশি HCL উৎপন্ন করবে। বেশি বেশি HCL উৎপন্ন হওয়ার কারণে GIT এর Usual (eg:duodenum) and Unusual site (eg: jejunum, Oesophagus) ulcer হবে যা সাধারণত standard ulcer therapy তে response করে না। কারণ excess gastrin parietal cell কে stimulate করার কারণে parietal cell mass বেড়ে যায় এবং বেশি বেশি HCL ক্ষরণ হয়।
বেশি বেশি HCL ক্ষরণ হবার কারণে pH এতোটাই কমে যায় যে pancreas এর Bicarbonate তা neutralize করতে পারে না। ফলে pancreatic enzyme এবং Bile salt কাজ করতে পারে না, এর ফলে খাদ্য digest না হওয়ার কারণে Diarrhoea এবং steatorrhea হয়।
Investigation :
1. Basal pH level of stomach: basal মানে খালি পেটে, আমরা জানি HCL খাবার পর বেশি ক্ষরন হয় কিন্তু খালি পেটে কম ক্ষরণ হয়।কিন্তু এখানে খালি পেটেও (Basal condition) বেশি ক্ষরণ হবে। কারণ excess gastrin। তাই Basal pH level ও অনেক কম পাবো।
2. Serum gastrin level: 10-1000 fold বেশি পাবো
3. CT scan and Endoscopic ulatrasound (Tumor localization এর জন্য)
Treatment :
1. PPI (High dose এ)
2. Somatostatin analogue : Octreotide
3. Surgery: resection of tumour
